ABSTRACT Introduction: Children with nephrotic syndrome are at increased risk of infections because of disease status itself and use of various immunosuppressive agents. In majority, infections trigger relapses requiring hospitalization with increased risk of morbidity and mortality. This study aimed to determine the incidence, spectrum, and risk factors for major infections in hospitalized children with nephrotic syndrome. Methods: All consecutive hospitalized children between 1-12 years of age with nephrotic syndrome were enrolled in the study. Children with acute nephritis, secondary nephrotic syndrome as well as those admitted for diagnostic renal biopsy and intravenous cyclophosphamide or rituximab infusion were excluded. Results: A total of 148 children with 162 admissions were enrolled. Incidence of major infections in hospitalized children with nephrotic syndrome was 43.8%. Peritonitis was the commonest infection (24%), followed by pneumonia (18%), urinary tract infection (15%), and cellulitis (14%), contributing with two thirds of major infections. Streptococcus pneumoniae (n = 9) was the predominant organism isolated in children with peritonitis and pneumonia. On logistic regression analysis, serum albumin < 1.5gm/dL was the only independent risk factor for all infections (OR 2.6; 95% CI, 1.2-6; p = 0.01), especially for peritonitis (OR 29; 95% CI, 3-270; p = 0.003). There were four deaths (2.5%) in our study, all due to sepsis and multiorgan failure. Conclusions: Infection remains an important cause of morbidity and mortality in children with nephrotic syndrome. As Pneumococcus was the most prevalent cause of infection in those children, attention should be paid to the pneumococcal immunization in children with nephrotic syndrome.


RESUMO Introdução: Crianças com síndrome nefrótica apresentam maior risco de infecções devido ao próprio status da doença e ao uso de vários agentes imunossupressores. Em grande parte, as infecções desencadeiam recidivas que exigem hospitalização, com risco aumentado de morbidade e mortalidade. Este estudo teve como objetivo determinar a incidência, o espectro e os fatores de risco para infecções graves em crianças hospitalizadas com síndrome nefrótica. Métodos: Todas as crianças hospitalizadas consecutivamente entre 1 e 12 anos de idade com síndrome nefrótica foram incluídas no estudo. Crianças com nefrite aguda, síndrome nefrótica secundária, bem como aquelas admitidas para biópsia renal diagnóstica e infusão intravenosa de ciclofosfamida ou rituximabe foram excluídas. Resultados: Foram cadastradas 148 crianças com 162 internações. A incidência de infecções graves em crianças hospitalizadas com síndrome nefrótica foi de 43,8%. A peritonite foi a infecção mais comum (24%), seguida por pneumonia (18%), infecção do trato urinário (15%) e celulite (14%), contribuindo com dois terços das principais infecções. Streptococcus pneumoniae (n = 9) foi o organismo predominantemente isolado em crianças com peritonite e pneumonia. Na análise de regressão logística, a albumina sérica < 1,5gm / dL foi o único fator de risco independente para todas as infecções (OR 2,6; 95% CI, 1,2-6; p = 0,01), especialmente para peritonite (OR 29; IC95% 3 -270, p = 0,003). Houve quatro mortes (2,5%) em nosso estudo, todas devido a sepse e falência de múltiplos órgãos. Conclusões: A infecção continua sendo uma importante causa de morbimortalidade em crianças com síndrome nefrótica. Como o Pneumococo foi a causa mais prevalente de infecção nessas crianças, deve-se atentar para a imunização pneumocócica em crianças com síndrome nefrótica.

Introducción: La Enfermedad Renal Crónica es un problema de salud mundial. Objetivo: Determinar el comportamiento de la mortalidad en pacientes con enfermedad renal en el Instituto de Nefrología durante 2016 y 2017. Material y Métodos: Estudio observacional descriptivo transversal de pacientes fallecidos con necropsia realizada. Se registraron variables demográficas, de laboratorio, las relacionadas con la terapia de reemplazo de la función renal y con el diagnóstico anatomo-patológico. Para cada una de las variables se calcularon frecuencias absolutas y relativas. En el caso de la variable presencia de sepsis en las defunciones, fue empleada la prueba de chi-cuadrado para la bondad del ajuste, para probar la hipótesis nula H0: de igualdad de la distribución de las tres categorías de la variable. Resultados: Se estudiaron 48 fallecidos con necropsia, 48,9 por ciento del total de fallecidos en el periodo. La correspondencia clínico patológica fue 80,1 por ciento. Predominó el sexo masculino, el grupo de edad superior a 60 años y la hipertensión arterial como antecedente. El método de terapia de reemplazo renal más documentado fue la hemodiálisis, mediante catéter venoso central un 87,8 por ciento. El 61,8 por ciento de los fallecidos recibieron tratamiento hemodialítico por 3 meses o menos. La sepsis fue la segunda causa de muerte precedida por eventos cardio y cerebrovasculares; no obstante, en el análisis de la totalidad de necropsias, la infección asociada directa o indirecta a la muerte tuvo una frecuencia mayor. Conclusiones: La infección directa o indirectamente asociada a la muerte, la hipoalbuminemia y el acceso vascular mediante catéter se relacionan con la mortalidad en el periodo(AU)


Introduction: Chronic kidney disease is a health problem worldwide. Objective: To determine the behavior of mortality in patients with kidney damage in the Nephrology Institute during the years between 2016 and 2017. Material and Method: A cross-sectional descriptive observational study was conducted in 48 deceased patients who underwent necropsy. Demographic and laboratory variables were recorded, as well as those related to the type of renal function replacement therapy and the anatomopathological diagnosis. Absolute and relative frequencies were calculated for each of the variables. In the case of the variable presence of sepsis in deaths, the Chi-Square Goodness-of-Fit Test was used to test the Null Hypothesis HO: uniformity of the distribution of the three categories of the variable. Results: A total of 48 deceased were studied at necropsy, representing 48.9 percent of the total number of deaths within the period. The clinical- pathological correlation was 80.1percent. The male sex, the group over the age of 60 years, and arterial hypertension as antecedent of the disease predominated in the study. The most documented method of renal function replacement therapy was hemodialysis which was performed through central venous catheter, 87.8 percent. The 61.8 percent of deceased patients received hemodialysis treatment for 3 months or less. Sepsis represented the second cause of death preceded by cardiovascular and cerebrovascular events. However, during the analysis of all the necropsies, the direct or indirect infection associated with the death had a greater frequency. Conclusions: Direct or indirect infections associated with death, hypoalbuminemia, and the vascular access with venous catheter was related to mortality during the observed period(AU)

ABSTRACT Objective: to evaluate the clinical outcome of elderly patients admitted to intensive care units who had nosocomial infection, correlating the findings with sociodemographic and clinical variables. Method: descriptive research, performed with 308 elderly patients. The collection was made from medical records and covers the years 2012 to 2015. Uni-/bivariate analyses were performed. Results: a statistical association was found between the clinical outcome types and the variables age, length of stay, presence of previous comorbidities, main diagnosis, respiratory and urinary tract infections, use of central venous and indwelling urinary catheters, mechanical ventilation, and tracheostomy. The survival curve showed higher mortality among the elderly from the age of 80 on. Conclusion: the clinical outcome of the elderly who acquire infection in the intensive care unit is influenced by sociodemographic and clinical variables that increase mortality rates.


RESUMEN Objetivo: evaluar el desenlace clínico de ancianos que adquirieron infección intrahospitalaria durante internación en Unidades de Terapia Intensiva, correlacionando los hallazgos con variables sociodemográficas y clínicas. Método: investigación descriptiva, realizada con 308 pacientes ancianos. Datos recolectados de historias clínicas, considerando los años de 2012 a 2015. Se realizaron análisis uni/bivariados. Resultados: se registró asociación estadística entre los tipos de desenlace clínico y las variables: faja etaria, tiempo de internación, presencia de comorbilidades previas, diagnóstico principal, infección del tracto respiratorio y urinario, uso de catéteres vesicales de demora y venoso central, ventilación mecánica y traqueotomía. La curva de supervivencia evidenció mayor mortalidad entre ancianos a partir de los 80 años. Conclusión: el desenlace clínico de ancianos que adquieren infecciones en Unidades de Terapia Intensiva es influenciado por variables sociodemográficas y clínicas, que incrementan las tasas de mortalidad.


RESUMO Objetivo: avaliar o desfecho clínico de idosos que adquiriram infecção hospitalar hospitalizados em Unidades de Terapia Intensiva, correlacionando os achados com variáveis sociodemográficas e clinicas. Método: pesquisa descritiva, realizada com 308 pacientes idosos. A coleta deu-se em prontuários e contempla os anos de 2012 a 2015. Realizaram-se análises uni-/bivariadas. Resultados: registrou-se associação estatística entre os tipos de desfechos clínicos e as variáveis: faixa etária, tempo de internação, presença de comorbidades prévias, diagnóstico principal, infecção do trato respiratório e urinário, uso de cateteres vesical de demora e venoso central, ventilação mecânica e traqueostomia. A curva de sobrevivência evidenciou maior mortalidade entre idosos a partir de 80 anos. Conclusão: o desfecho clínico de idosos que adquirem infecção na Unidade de Terapia Intensiva é influenciado por variáveis sociodemográficas e clínicas, que incrementam as taxas de mortalidade.

ABSTRACT

Objetivo: Comprobar el impacto de las complicaciones infecciosas postoperatorias en la sobrevida a largo plazo de pacientes gastrectomizados por carcinoma gástrico con intención curativa. Materiales y métodos: El presente estudio de cohortes evaluó una serie de 79 pacientes con diagnóstico de carcinoma gástrico avanzado resecable con intención curativa. Se agruparon a su vez: Grupo A (N=28): Pacientes con complicaciones infecciosas postoperatorias y Grupo B (N=51): Pacientes que no presentaron complicaciones infecciosas postoperatorias. El estudio abarcó los años 2008-2013. Resultados: En el grupo A, las tasas de sobrevida a 1, 3 y 5 años fueron de 74%, 74% y 47,6% respectivamente. En el grupo B, las tasas de sobrevida a 1, 3 y 5 años fueron de 80,3%, 56% y 49,8% respectivamente (p=0,365). Las principales complicaciones infecciosas postoperatorias no relacionadas con la técnica quirúrgica fue la neumonía (20,3%), seguida de infección del tracto urinario (3,8%). Las principales complicaciones infecciosas postoperatorias relacionadas con la técnica quirúrgica fue la sepsis (n=5), dos de estos pacientes presentaron de dehiscencia de anastomosis esofagoyeyunal, dos presentaron fístula gastroyeyunal, uno fístula enterocutánea y por último un paciente presentó absceso y necrosis de tejido peripancreático. Conclusiones: No hubo un impacto en la sobrevida a 5 años en pacientes con complicaciones infecciosas postoperatorias post gastrectomía con intención curativa. Sin embargo, más estudios adicionales deberían efectuarse.


Objective: To determine the impact of postoperative infectious complications in the long term survival of patients gastrectomized for gastric carcinoma with curative intent. Materials and methods: The present cohort study evaluated a series of 79 patients diagnosed with resectable advanced gastric carcinoma with curative intent. They were grouped in: Group A (N=28): patients with postoperative infectious complications and Group B (N=51): patients who did not develop postoperative infectious complications. The study covered the years 2008-2013. Results: In group A, the survival rates at 1, 3 and 5 years was 74%, 74% and 47.6% respectively. In group B, the survival rates at 1, 3 and 5 years was 80.3%, 56% and 49.8% respectively (p=0.365). The main postoperative infectious complications not related to the surgical technique were pneumonia (20.3%), followed by urinary tract infection (3.8%). The main postoperative infectious complications related to surgical technique was sepsis (n=5), two of which were related to dehiscence esophagojejunal anastomosis, two bye gastroyeyunal fistula, another bye enterocutaneous fistula and one patient who presented abscess and necrosis peripancreatic’s tissue. Conclusions: There was no impact on survival at 5 years in patients with postoperative infectious complications after gastrectomy with curative intent. However, further studies should be carried over.

INTRODUÇÃO: As Infecções de Sítio Cirúrgico (ISCs) são as complicações mais freqüentes que ocorrem nos pacientes após as cirurgias e são responsáveis pela elevação da morbidade, mortalidade e dos custos hospitalares. OBJETIVO: O objetivo deste estudo foi estimar o custo direto adicional associado às ISCs ocorridas no período de 2011 a 2013 em um Hospital Universitário de Salvador, Bahia. Para tanto buscou-se caracterizar a população de pacientes acometidos por ISC, segundo os aspectos sócio-demográficas, condições clínicas e cirurgias realizadas, realizar uma revisão integrativa atualizada da literatura mundial sobre o custo dessas infecções e analisar os custos associados aos cuidados à saúde dos pacientes cirúrgicos segundo presença de ISC. METODOLOGIA: Trata-se se um estudo epidemiológico do tipo caso-controle pareado, realizado com informações dos registros hospitalares dos pacientes. Foram incluídos como população do estudo todos os casos de ISCs em cirurgias eletivas e limpas. Os controles foram pareados por idade, sexo e tipo de cirurgia realizada respeitando o princípio da similaridade. As proporções foram comparadas por meio dos testes 2 e exato de Fisher quando adequados com nível de 5% de significância estatística. Para estimar as diferenças das médias de custos utilizou-se o modelo de regressão linear. RESULTADOS: No total foram selecionados 259 pacientes. Os casos de ISC ocorreram predominantemente em mulheres, com idade entre 61 a 75 anos. Os principais fatores atribuídos aos custos com as ISCs foram o uso de antibióticos para o seu tratamento, a internação em Unidade de Terapia Intensiva, a realização de exames e reabordagens cirúrgicas. No geral, o custo médio hospitalar em pacientes com ISC foi aproximadamente o dobro do valor aferido daqueles não infectados. CONCLUSÕES: Foi evidenciado neste estudo a necessidade de reforçar o desenvolvimento constante de ações preventivas e de controle das ISCs a fim de garantir a segurança na assistência prestada aos pacientes cirúrgicos e conseqüentemente a redução dos custos atribuídos a essa complicação para o hospitais e sistema de saúde.


INTRODUCTION: The Surgical Site Infections (SSI) are the most frequent complications occurring in patients after surgery and are responsible for high morbidity, mortality and hospital costs. OBJECTIVE: The aim of this study was to estimate the additional direct costs associated with SSIs occurred in the 2011-2013 period at a University Hospital in Salvador, Bahia. For that sought to characterize the population of patients affected by ISC, according to the socio-demographic aspects, clinics and surgeries conditions, carry out a integrative and updated review of the literature on the cost of these infections and analyze the costs associated with health care to patients surgical according to the presence of ISC. METHODOLOGY: This is an epidemiological study of the case-control matched, conducted with information from the hospital records of patients. They were included in the study population all cases of ISCs in elective surgery and clean. The controls were matched for age, gender and type of surgery performed respecting the principle of similarity. Proportions were compared using the Fisher's exact test and χ2 when appropriate and at 5% statistical significance. To estimate the differences of the average costs used the linear regression model. RESULTS: In total 259 patients were selected. The cases of ISC occurred primarily in women between 61-75 years. The main factors attributed to the costs of the ISCs were the use of antibiotics for their treatment, to stay in the Intensive Care Unit, conducting exams and news surgical interventions. Overall, the average hospital cost in patients with SSI was approximately twice with those not infected. CONCLUSIONS: It was shown in this study the need of to reinforce the constant development of preventive and control of SSIs in order to secure the assistance provided to surgical patients and therefore reducing costs attributed to this complication for hospitals and health care system.

OBJECTIVE: To characterize the deaths of 193 children with sickle cell disease screened by a neonatal program from 1998 to 2012 and contrast the initial years with the final years. METHODS: Deaths were identified by active surveillance of children absent to scheduled appointments in Blood Bank Clinical Centers (Hemominas). Clinical and epidemiological data came from death certificates, neonatal screening database, medical records, and family interviews. RESULTS: Between 1998 and 2012, 3,617,919 children were screened and 2,591 had sickle cell disease (1:1,400). There were 193 deaths (7.4%): 153 with SS/Sß0-talassemia, 34 SC and 6 Sß+thalassemia; 76.7% were younger than five years; 78% died in the hospital and 21% at home or in transit. The main causes of death were infection (45%), indeterminate (28%), and acute splenic sequestration (14%). In 46% of death certificates, the term "sickle cell" was not recorded. Seven-year death rate for children born between 1998 and 2005 was 5.43% versus 5.12% for those born between 2005 and 2012 (p = 0.72). Medical care was provided to 75% of children; 24% were unassisted. Medical care was provided within 6 hours of symptom onset in only half of the interviewed cases. In 40.5% of cases, death occurred within the first 24 hours. Low family income was recorded in 90% of cases, and illiteracy in 5%. CONCLUSIONS: Although comprehensive and effective, neonatal screening for sickle cell disease was not sufficient to significantly reduce mortality in a newborn screening program. Economic and social development and increase of the knowledge on sickle cell disease among health professionals and family are needed to overcome excessive mortality. .


OBJETIVO: Caracterizar os 193 óbitos de crianças com doença falciforme diagnosticadas por programa de triagem neonatal entre 1998-2012 e comparar os primeiros com os últimos anos. MÉTODOS: Os óbitos foram identificados pela busca ativa das crianças ausentes nas consultas agendadas nos hemocentros. Dados clínicos e epidemiológicos provieram dos documentos de óbito, banco de dados da triagem neonatal, prontuários médicos e das entrevistas com parentes. RESULTADOS: Entre 1998-2012 foram triadas 3.617.919 crianças, 2.591 com doença falciforme (1:1.400). Ocorreram 193 óbitos (7,4%): 153 com SS/Sß0-talassemia, 34 SC e 6 Sß+-talassemia; 76,7% em crianças com menos de cinco anos; 78% faleceram em hospitais e 21% em domicílio ou trânsito. Causas principais do óbito: 45% infecção, 28% indeterminada, 14% sequestro esplênico agudo. Em 46% dos documentos de óbito, não houve registro do termo "falciforme". A taxa de mortalidade até sete anos das crianças nascidas entre 1998-2005 foi 5,43% versus 5,12%, entre 2005-2012 (p = 0,72). Receberam assistência médica 75% das crianças; 24% ficaram desassistidas. Pelas entrevistas, atendimento médico teria ocorrido nas primeiras seis horas do início dos sintomas em metade dos casos. O óbito ocorreu em 40,5% dos casos, nas primeiras 24 horas. Baixa renda familiar foi registrada em 90% dos casos e analfabetismo em 5%. CONCLUSÕES: A triagem para doença falciforme, mesmo abrangente e eficaz, não foi suficiente para reduzir significativamente a mortalidade no Programa de Triagem Neonatal. Necessita-se de desenvolvimento econômico e social do Estado e ampliação, pela educação continuada, do conhecimento sobre a doença falciforme entre os profissionais de saúde e parentes. .

Eosinopenia, a biomarker for infection, has recently been shown to be a predictor of adult mortality in the intensive care unit (ICU). Our study assessed the usefulness of eosinopenia as a mortality and an infection biomarker in the pediatric ICU (PICU). We compared the PICU mortality scores, eosinophil count and percentage at ICU admission between children who survived and those who did not survive and between children with infection and those without infection. A total of 150 patients were evaluated. The initial eosinophil count and percentage were significantly lower in the group that did not survive when compared to those that did survive (P < 0.001; P < 0.001). However, there was no significant difference in the eosinophil count and percentage seen in patients with and without infection. Eosinopenia, defined as an eosinophil count < 15 cells/microL and an eosinophil percentage < 0.25%, (hazard ratio [HR]: 2.96; P = 0.008) along with a Pediatric Index of Mortality (PIM) 2 (HR: 1.03; P = 0.004) were both determined to be independent predictors of mortality in the PICU. The presence of eosinopenia at the ICU admission can be a useful biomarker for mortality in children, but is not useful as a biomarker for infection.

Introducción. Los mayores problemas de mortalidad en menores de cinco años se presentan en países con altos índices de pobreza, lo que demuestra la iniquidad en relación con los países desarrollados. Se han identificado causas generales de mortalidad, como trastornos perinatales, sarampión, VIH/ sida, diarrea e infecciones respiratorias agudas, por lo que se hace énfasis en que la desnutrición ha tenido que ver con la mitad de ellas, aproximadamente, como una causa subyacente. Objetivo. Caracterizar la mortalidad por desnutrición utilizando un método de ajuste para la tasa, dadas las dificultades en su estimación por las limitaciones en la identificación de la desnutrición como causa básica de muerte. Materiales y métodos. Se llevó a cabo un estudio descriptivo que incluyó análisis de códigos CIE- 10 de las estadísticas vitales entre 2003-2007, con estimación de las tasas de mortalidad, aplicación del método de causas múltiples para enfermedades infecciosas y cálculo de las probabilidades de muerte. Resultados. La desnutrición, como antecedente, tiene asociación con las enfermedades infecciosas; la frecuencia de la enfermedad infecciosa como causa básica de muerte fue siete veces mayor cuando coexistió la desnutrición como causa antecedente. Una vez ajustada la tasa de mortalidad, se encontró que aumentó cinco veces el valor inicial. El método de "análisis de causas múltiples" establece su efectividad como metodología en el ajuste de este tipo de mortalidad. Conclusión. La desnutrición puede ser una causa básica o subyacente de muerte en una de cada ocho defunciones, en niños menores de un año, y en una de cada tres, en niños de uno a cuatro años.


Introduction. Much higher mortalities occur in children under five in developing countries with high poverty rates compared with developed countries. Causes of death are related to perinatal conditions, measles, HIV/AIDS, diarrhea, respiratory diseases and others. Throughout the world, malnutrition has been identified as the underlying cause of approximately half of these deaths. Objective. Death rate due to malnutrition was described using an adjusted method that takes into account the difficulties of identifying malnutrition as a direct cause of death. Materials and methods. A descriptive study included analysis of the International Classification of Diseases (ICD-10) vital statistics from 2003-2007. Death rates were estimated, a method of analysis of multiple causes was applied for infectious diseases, along with calculations of death probabilities. Results. Malnutrition was associated with infectious diseases. The frequency of infectious disease as a direct cause of death was almost seven times higher in cases with the antecedent of malnutrition. When adjusted death rate values were used, the initial value increased nearly five times. The probability of death after the adjustment for inadequate classification increased approximately four times. The Analysis of Multiple Causes Method was established as an effective method in analyzing malnutrition and infectious diesease mortality in Colombia. Conclusion. Malnutrition may be a direct underlying cause of death in one of eight deaths in children <1 year old and one of three deaths in 1-4-year-olds.

ABSTRACT

Objective: To correlate the Apgar score, and neonatal mortality and its causes at a hospital located in the southern area of São Paulo City. Methods: A retrospective study performed by analysis of medical charts (n=7,094) of all live newborns during the period of 2005 to 2009, with data up to 28 days of life in reference to weight, Apgar score, survival and cause of mortality. Cases were analyzed by the X² test (p < 0.05). Results: In 7,094 births, there were 139 deaths, 58.3% during the first week, and 3.6% of them with Apgar < 4 in the 1st minute. A positive association was found between mortality and this variable, with significantly declining values up to 2,000 g in weight. In the group with weight < 1,000 g, the association with Apgar < 4 in the 1st minute with mortality was three-fold greater than in the 1,000-1,500 g weight group, and 35-fold greater than in the ? 3,000 g group. Among newborns with Apgar 8-10, the rate of mortality and low weight was two times greater than in those with weight > 2,499 g. Fetal distress and prematurity were associated with early neonatal death; malformations and fetal distress to late mortality. The predictive value of death with Apgar < 4 varied, according to weight, from 62.74% in the < 1,000 g group to 5.5%, in the > 3,000 g group. Conclusions: The Apgar score proved linked to factors both epidemiological and related to attention given to the birth and neonatal mortality, and was associated with extremely low birth weight.


Objetivo: Correlacionar o escore de Apgar e a mortalidade neonatal e suas causas em um hospital localizado na zona Sul do município de São Paulo. Métodos: Estudo retrospectivo por análise de prontuário (n=7.094), de todos os recém-nascidos vivos, no período de 2005 a 2009, com dados referentes até os 28 dias de vida, quanto a peso, escore de Apgar, sobrevida e causa de mortalidade. Os casos foram analisados pelo teste do X² (p < 0,05). Resultados: Nos 7.094 nascimentos, houve 139 óbitos, 58,3% na primeira semana, 3,6% com Apgar < 4 no 1º minuto. Foi encontrada associação positiva entre mortalidade e essa variável, com valores decrescentes significantemente até o peso de 2.000 g. No grupo de peso < 1.000 g, a associação do Apgar < 4 no 1º minuto com mortalidade foi três vezes maior do que no grupo 1.000 a 1.500 g e 35 vezes maior do que no grupo ? 3.000 g. Entre os recém-nascidos com Apgar de 8 a 10, a mortalidade entre baixo peso foi duas vezes maior do que nos de peso > 2.499 g. O sofrimento fetal e a prematuridade se associaram a óbito neonatal precoce; malformações e o sofrimento fetal à mortalidade tardia. O valor preditivo de morrer quando o Apgar < 4 variou, conforme o peso, entre 62,74% no grupo < 1.000 g a 5,5% no grupo > 3.000 g. Conclusões: O escore de Apgar se mostrou ligado a fatores epidemiológicos e de atenção ao parto, à mortalidade neonatal e se associou a extremo baixo peso.

ABSTRACT

Metabolic abnormalities are common throughout the course of human immunodeficiency virus (HIV) infection and may occur either due to HIV infection or as a result of side effects of antiretroviral therapy. It has been established that dyslipidemia and dysglycemia associated with HIV disease reduce the long-term survival of the patients; but their role for predicting prognosis of short-term mortality in HIV patients is unknown. Aim: To study dyslipidemia and dysglycemia as a prognostic indicator for short-term mortality (3 months) in HIV patients. Subjects and Methods: An observational; prospective study was conducted at a tertiary care center over a period of 6 months. Consecutive HIV-positive patients hospitalized (both; HIV status known prior to hospitalization and the diagnosis made for the first time at admission) in medical wards from March to May 2010 were studied. All patients had their random blood sugars; fasting blood sugars (if possible); fasting lipid profile; and cluster of differentiation 4 (CD4) counts tested at the time of enrollment. The patients were followed for a period of 3 months; at the end of which they were categorized as survivors and non-survivors; and the demographic; clinical; and investigational parameters were compared between the above groups. Data was analyzed by applying Mann-Whitney U test; two sample t-test; Fisher-Exact test; and stepwise logistic regression analysis of significance; using the computer-based program; Stata; version 11.1. Results: A total of 82 patients were enrolled for the study of which 64 (78.05) were males and 18 (21.95) were females; with a mean (SD) age of 34.00 (7.0) years. The mean CD4 count was 206.23 (129.5) cells/mm 3 . The overall mortality within 3 months was 20.7(17/82). ABSTRACT

OBJECTIVE: To identify preoperative and transoperative risks factors for postoperative complications developed in lung resection surgery. METHODS: During 14 months; 189 patients underwent pulmonary resection and were enrolled to the study. After a clinical interview, patients were evaluated by laboratory, pulmonary function tests and radiography, submitted to a surgical procedure, and were followed during their stay in the ICU and hospital, evaluating postoperatory complications and death. RESULTS: The postoperative rate of complications was 52.9 percent: respiratory (34.3 percent), infectious (31 percent), and cardiovascular (21.4 percent). Respiratory complications were related to smoking (p < 0.01, RR 2.31), airway obstruction by spirometry (p = 0.01, RR 2.60), presence of anemia (p < 0.01, RR 2.13), and prolonged protrombine time [PT] (p = 0.03, RR 1.77). Infection complications were related to smoking (p < 0.01, RR 2.69), airway obstruction by spirometry (p = 0.01, RR 3.31), presence of anemia (p < 0.01, RR 2.10), and prolonged PT (p = 0.03, RR 2.29). Cardiovascular problems were related with older age (p < 0.01, RR 2.66), cigarette smoking (p < 0.01, RR 4.55), and hypoxemia (p = 0.03, RR 2.43). The postoperative mortality rate was 7.1 percent. CONCLUSION: A preoperative evaluation can provide a suitable and safe postoperative prediction of complications in patients submitted to lung resection. Patients with COPD, hypoxemic, older, and anemic patients must be classified as high-risk for developing these complications.


OBJETIVO: Identificar os fatores de risco pré e transoperatórios para o desenvolvimento de complicações pós-operatórias na cirurgia de ressecção pulmonar. INTRODUÇÃO: Os pacientes submetidos à cirurgia de ressecção pulmonar desenvolvem graves e frequentes complicações pós-operatórias. A identificação dos fatores de risco para o desenvolvimento das mesmas é fundamental na predição das complicações no pós-operatório. MÉTODOS: Durante 14 meses, 189 pacientes foram submetidos à intervenção cirúrgica torácica e foram incluídos no estudo. Depois de uma entrevista clínica, os pacientes foram avaliados por exames laboratoriais, espirometria e exames de imagem. Os mesmos foram submetidos ao procedimento cirúrgico e foram seguidos durante a sua permanência na UTI e no hospital, avaliando as complicações pós-operatórias e o risco de morte. RESULTADOS: A taxa de complicações pós-operatórias foi de 52,9 por cento, principalmente respiratórias (34,3 por cento), infecciosas (31 por cento) e cardiovasculares (21,4 por cento). As complicações respiratórias foram relacionadas ao tabagismo (p < 0,01, RR 2,31), obstrução das vias aéreas (p = 0,01, RR 2,60), presença de anemia (p < 0,01, RR 2.13), e prolongado tempo de protrombina [PT] (p = 0,03, RR 1,77). As complicações infecciosas estiveram relacionados ao tabagismo (p < 0,01, RR 2,69), obstrução de vias aéreas (p = 0,01, RR 3,31), presença de anemia (p < 0,01, RR 2.10) e TP prolongado (p = 0,03, RR 2,29 ). Os problemas cardiovasculares, especialmente a presença de arritmias, foram relacionados com idade mais avançada (p < 0,01, RR 2,66), tabagismo (p < 0,01, RR 4,55) e hipoxemia (p = 0,03, RR 2,43). A taxa de mortalidade pós-operatória foi de 7,1 por cento. CONCLUSÃO: A identificação dos fatores de risco pode predizer complicações pós-operatórias nos pacientes submetidos a ressecção pulmonar. Pacientes com DPOC, hipoxêmicos, idosos e anêmicos devem ser classificados como de alto risco para o desenvolvimento de complicações.

ABSTRACT

Heart transplantation is a standard treatment for end-stage heart disease. Pediatric heart transplantation, however, is not frequently performed due to the shortage of pediatric heart donors. This is the first report of pediatric heart transplantation in Korea. Our retrospective study included 37 patients younger than 18 yr of age who underwent heart transplantation at Asan Medical Center between August 1997 and April 2009. Preoperative diagnosis was either cardiomyopathy (n = 29, 78.3%) or congenital heart disease (n = 8, 22.7%). Mean follow up period was 56.9 +/- 44.6 months. There were no early death, but 7 late deaths (7/37, 18.9%) due to rejection after 11, 15, 41 months (n = 3), infection after 5, 8, 10 months (n = 3), suspicious ventricular arrhythmia after 50 months (n = 1). There was no significant risk factor for survival. There were 25 rejections (25/37, 67.6%); less than grade II occurred in 17 patients (17/25, 68%) and more than grade II occurred in 8 patients (8/25, 32%). Actuarial 1, 5, and 10 yr survival was 88.6%, 76.8%, and 76.8%. Our midterm survival of pediatric heart transplantation showed excellent results. We hope this result could be an encouraging message to do more pediatric heart transplantation in Korean society.

ABSTRACT

Heart transplantation is now regarded as the treatment of choice for end-stage heart failure. To improve long-term results of the heart transplantation, we analyzed causes of death relative to time after transplantation. A total of 201 consecutive patients, 154 (76.6%) males, aged > or = 17 yr underwent heart transplantation between November 1992 and December 2008. Mean ages of recipients and donors were 42.8 +/- 12.4 and 29.8 +/- 9.6 yr, respectively. The bicaval anastomosis technique was used since 1999. Mean follow up duration was 6.5 +/- 4.4 yr. Two patients (1%) died in-hospital due to sepsis caused by infection. Late death occurred in 39 patients (19.4%) with the most common cause being sepsis due to infection. The 1-, 5-, and 10-yr survival rates in these patients were 95.5% +/- 1.5%, 86.9% +/- 2.6%, and 73.5% +/- 4.1%, respectively. The surgical results of heart transplantation in adults were excellent, with late mortality due primarily to infection, malignancy, and rejection. Cardiac deaths related to cardiac allograft vasculopathy were very rare.

Introdução: Complicações infecciosas determinam significativas morbidade e mortalidade após transplante renal. A imunossupressão utilizada representa o principal fator de risco e apresenta relação direta com a incidência e a severidade dos eventos infecciosos. Métodos: estudo de coorte, retrospectivo, que analisou a incidência e fatores de risco para ocorrência de infecções em 1.676 receptores de transplante renal durante o primeiro ano de acompanhamento. Resultados: Eventos infecciosos foram observados em 821 (49%) pacientes. O número médio de episódios infecciosos entre pacientes com pelo menos um episódio foi de 2,3 (1 - 12). As complicações infecciosas mais prevalentes foram infecção do trato urinário (31,3%), infecções por citomegalovírus (12%), infecção da incisão cirúrgica (10,3%), infecção por herpes vírus (9,1%), infecção pulmonar (5,2%) e infecção da corrente sanguínea (4,3%). O tempo de isquemia fria e a utilização de rins de doadores falecidos foram fatores de risco importantes para ocorrência de episódios infecciosos. Conclusões: Infecções apresentam prevalência elevada no primeiro ano de acompanhamento após o transplante. A principal complicação infecciosa foi a infecção do trato urinário.


Introduction: Infectious complications determine significant morbidity and mortality after renal transplantation. Immunosuppression used represents the main risk factor and has direct relation with the incidence and severity of infectious events. Methods: A cohort study, retrospective study examined the incidence and risk factors for occurrence of infections in 1676 renal transplant recipients during the first year of monitoring. Results: infectious events were observed in 821 (49%) patients. The average number of infectious episodes among patients with at least one episode was 2.3 (1-12). Infectious complications were most prevalent urinary tract infection (31.3%), cytomegalovirus infections (12%), infection of the surgical incision (10.3%), herpes virus infection (9.1%), pulmonary infection ( 5.2%) and bloodstream infection (4.3%). The cold ischemia time and the use of kidneys from deceased donors were significant risk factors for the occurrence of infectious episodes. Conclusions: Infections have a high prevalence in the first year of monitoring after transplantation. The main infectious complication was urinary tract infection.

En el año 2002 la Organización de Naciones Unidas anunciaba que a mediados del siglo XXI la población de la tercera edad superaría a los jóvenes. Nuestro objetivo es determinar la morbimortalidad en cirugía abdominal de urgencia en el adulto mayor, a través de un estudio descriptivo y prospectivo. Se analizaron los pacientes de 60 años o más operados de cirugía abdominal de urgencia en el período comprendido de enero 2005 a enero del 2008. El grupo etario predominante fue el comprendido entre 70 y 79 años con 42 pacientes. La cirugía abdominal urgente representó el 23.1 por ciento del total de la cirugía abdominal en los ancianos. Los principales factores de riesgo fueron las enfermedades de origen cardiovascular 35.2 por ciento. El 41.0 por ciento tenía más de un factor de riesgo. Predominaron los clasificados como ASA II 57.5 por ciento. Las hernias complicadas de la pared abdominal fue la enfermedad de mayor incidencia 42.2 por ciento. La mortalidad global de la serie fue del 20 por ciento.


In 2002 the United Nations Organization announced that at around the middle of the XXI century the elder population will surpass the young one. Our objective was determining the morbimorbitality of the emergency abdominal surgery in elderly people, carrying out a prospective descriptive study. We analyzed the sixty-years-old and more patients operated of emergency abdominal surgery in the period from January 2005 to January 2008. The predominant age group was the one between 70 and 79 years old, with 42 patients. The emergency abdominal surgery represented the 23.1 percent of the total of the abdominal surgeries in elderly people; the main risk facts were diseases of cardiovascular origin: 35.2 percent. 41.0 percent had more than one risk fact. There was a predominance of patients classified as ASA II (57.5 percent). The disease with the highest incidence was complicated abdominal hernias (42.2 percent). The series global mortality was 20 percent.

A Leishmaniose visceral (LV) tem vasta distribuição geográficas nas áreas tropicais e subtropicais do planeta, cujo parasito é um protozoário do gênero Leishmania. Visceral leishmaniasis (VL) has a wide geographical distribution in tropical and subtropical areas of the planet, which is a protozoan parasite of the genus Leishmania. ABSTRACT

A retrospective study was performed on 169 beef and dairy calves aged from 1 to 7 days old submitted to the Diagnostic Laboratories at INTA Balcarce, Argentina. Bacterial culture was performed for aerobic and microaerophilic organisms. Samples from spleen and lymph nodes, and peripheral blood mononuclear cells were also cultured for viral isolation on cell culture. Bovine rotavirus was detected by direct-ELISA. Multiple tissue samples were fixed in 10% formalin, routinely processed and stained with hematoxylin and eosin for microscopic examination. Etiological diagnosis was made in 70 of the 169 calves. Infectious agents were identified in 49 cases, the most common being Escherichia coli. When the histopathological examination was performed in cases with undetermined diagnosis, it was noted that 44 specimens had histological lesions, which suggested the presence of an infectious agent. In order to characterize the causes of bovine neonatal mortality, the protocols and methodology should be improved in further works.


Se realizó un estudio restrospectivo en 169 terneros muertos 1 a 7 días después del nacimiento pertenecientes a rodeos para carne y leche, remitidos a los Laboratorios de Diagnóstico del INTA Balcarce, Argentina. Para detectar organismos aeróbicos y microaerófilos se realizó el cultivo bacteriano. Para el aislamiento viral sobre cultivo celular, se recolectaron muestras de bazo, ganglios linfáticos y sangre periférica. El rotavirus bovino fue identificado por ELISA directo. Se efectuó el examen microscópico de diferentes tejidos, los cuales fueron fijados en formol al 10%, procesados y teñidos con hematoxilina y eosina. Se obtuvo un diagnóstico etiológico en 70 de los 169 terneros. Se identificaron agentes infecciosos en 49 casos, siendo el más común Escherichia coli. En los casos con diagnóstico indeterminado, el examen histopatológico realizado determinó que 44 especímenes poseían lesiones compatibles con la presencia de agentes infecciosos. Es necesario mejorar los protocolos y las metodologías de trabajo a los fines de caracterizar las causas de mortalidad neonatal en bovinos.

In vitro evidence shows that immune function is compromised in people with diabetes. Therefore, infections are more common and infection-related mortality is higher in this group. Accurate information about the prevalence of infectious diseases in diabetic patients can help in health care policy and support planning. We do not know the spectrum of clinical infectious diseases in our patients. This study was conducted to determine the most common infectious diseases in diabetic patients. In this cross-sectional study all diabetic patients who were admitted to our hospital in Zahedan [Southeastern Iran] between 2001 and 2006, were evaluated. At first, we selected all of the patients with diabetes. Then all patients files were reviewed retrospectively for demographic factors and duration of diabetes, type of diabetes, cause of admission, prevalence of infectious diseases, duration of hospitalization and mortality rate. A total of 112 patients with diabetes [45 males and 67 females with an age range of 10 to 80] were evaluated. Pneumonia, diabetic foot and tuberculosis were the most common infections. Seventy percent of cases had type 2 diabetes. Duration of hospitalization was between 1 to 20 days. Upon the results emerged from this study, infection is the most frequent cause of hospitalization and death in the diabetic patients. Control of the disease, good health care system and vaccination can decrease the rate of infectious diseases

JUSTIFICATIVA E OBJETIVOS: Determinar a prevalência de infecções adquiridas em UTI e os fatores de risco para estas infecções, identificar os organismos infectantes mais prevalentes, avaliar a relação entre infecção adquirida na UTI e mortalidade. MÉTODO: Estudo de prevalência de um dia. Participaram do estudo 16 UTI do estado do Rio Grande do Sul, excluindo unidades coronarianas ou pediátricas. Todos os pacientes com idade maior que 12 anos, ocupando um leito de UTI por um período de 24h, foram incluídos. As 16 UTI coletaram dados de 174 pacientes. Principais desfechos: taxas de infecção adquirida na UTI, padrões de resistência dos patógenos isolados e fatores potenciais de risco para infecção adquirida na UTI e mortalidade. RESULTADOS: Um total de 122 pacientes (71 por cento) estava infectado, e 51 (29 por cento) adquiriram infecção na UTI. Pneumonia (58,2 por cento), infecção do trato respiratório inferior (22,9 por cento), infecção do trato urinário (18 por cento) foram os tipos mais freqüentes de infecção. Os microorganismos mais relatados foram stafilococos aureus (42 por cento [64 por cento resistentes a oxacilina]) e pseudomonas aeruginosa (31 por cento). Seis fatores de risco foram identificados para infecção adquirida na UTI: cateter urinário, acesso vascular central, intubação traqueal por tempo prolongado (> 4 dias), doença crônica, trauma e internação prolongada na UTI (> 30 dias). Os fatores de risco associados à morte foram idade, APACHE II, falência orgânica e prótese em via aérea com ou sem ventilação mecânica. CONCLUSÕES: A infecção adquirida na UTI é comum e freqüentemente associada a isolados de microorganismos resistentes. Este estudo, apesar de sua abrangência regional, serve de referência epidemiológica para ajudar a programar políticas de controle de infecção.


BACKGROUND AND OBJECTIVES: To determine the prevalence of intensive care unit (ICU)-acquired infections and the risk factors for these infections, identify the predominant infecting organisms, and evaluate the relationship between ICU-acquired infection and mortality. METHODS: A 1-day point prevalence study. Sixteen ICU of the State of Rio Grande do Sul-Brazil, excluding coronary care and pediatric units. All patients < 12 yrs occupying an ICU bed over a 24-hour period. The 16 ICU provided 174 case reports. Main outcomes: rates of ICU-acquired infection, resistance patterns of microbiological isolates, and potential risks factors for ICU-acquired infection and death. RESULTS: A total of 122 patients (71 percent) was infected and 51 (29 percent) had ICU-acquired infection. Pneumonia (58.2 percent), lower tract respiratory infection (22.9 percent), urinary tract infection (18 percent) were the most frequents types of ICU infection. Most frequently microorganisms reported were staphylococcus aureus (42 percent [64 percent resistant to oxacilin]) and pseudomonas aeruginosa (31 percent). Six risk factors for ICU acquired infection were identified: urinary catheterization, central vascular line, tracheal intubation for prolonged time (> 4 days), chronic disease and increased length of ICU stay (> 30 days). The risks factors associated with death were age, APACHE II, organ dysfunction, and tracheal intubation with or without mechanical ventilation. CONCLUSIONS: ICU-acquired infection is common and often associated with microbiological isolates of resistant organisms. This study may serve as an epidemiological reference to help the discussion of regional infection control policies.
